The Month from Hell is over, and John has the weekend off (first time in over a month), and his first real (i.e. not postcall) day off in almost three weeks. In hospital doublethink, postcall days count as days off. In what other line of work can your employer claim that you had the day "off" when you were on duty from midnight to 7:00 am? It's legal, but I don't have to like it. Assholes!
It's been about a year since Johns Hopkins got a slap on the wrist, and I was curious about what's happened since they got their reprieve. I suppose it's their right to try covering their asses, but it's also my right to point out the weasel words in their spin campaign.
Last year, the ACGME announced that it would begin enforcing tough resident duty-hour standards on July 1, 2003. These restrict residents' time in hospitals to 80 hours a week averaged over four weeks, and require three days off between on-call duties, 10 hours off between shifts and one day off out of seven....[Emphasis mine]
The ACGME calls for "in-house call no more than once every three nights, averaged over four weeks." This is Q3, which means that a call shift begins three days after your last call shift began: you're on call Monday night, your post-call "day off" is Tuesday, you work a regular shift on Wednesday, and are on call again Thursday. Calling this "three days off" is a disgusting bit of Newspeak. I suppose it all depends what your definition of "off" is...but they make it sound like you have 72 duty-free hours postcall to sit on the couch, eat bon-bons and watch soap operas. Doubleplus ungood assholes!

My wife had her worst call night ever saturday; the attending on call was only on call the one night out of the month, and had scheduled everything last night, so she was doing operations back-to-back all day and all night.

Operating all night is one of the best parts of being a surgical residents. It beats the hell out of answering stupid floor calls all night. She should kiss her attending.
Of course one of the other great parts of being a surgical resident is bitching about how bad your call night was.
