Well, the way I see it is that there is really no marginal cost to making residents work like slaves, while ther is a huge marginal benefit to squeezing every drop of work out of them as possible.
Think about the fact, that the cost of even getting a PA to pick up the slack from a resident only workling 40 hours would cost somewhere close to $120,000. On top of that, if you treat a PA poorly, they will walk, and then you are losing even more money.
When compared to the $35K a year that a resident cost to do more than twice the work of a PA, the amount of money being made is ridiculous.
I remember some idiot pedi resident telling me how the match was really the best thing going, and how it was a mistake to try to introduce competition for spots in the form of offering a just compensation.
His argument was basically that bad programs would end up closing, and that there would be fewer spots available. My retort was that it would mean that all programs would have a vested interest in maximizing their return by creating good programs.
Anyway, the 80 hour workweek is still a myth in many ways, but there is a reason for anonymous reporting.
